End of life/advance care planning, or health care directive, provides people with tools to control their healthcare decisions as well as other important issues while they can still take part in the decision-making process. It helps ensure that those who care for them can carry out their last wishes. While it may be a difficult subject to consider and discuss, it is important for a person to have their affairs in order to help facilitate a smooth process, and this can offer peace of mind to both the person and those responsible for carrying out their wishes.
Five Wishes is both a tool to help make these decisions and a legal document, ensuring these will be carried out at the end of life or in any medical situation where you’re unable to speak for yourself.
